Randomized Clinical Trial of Intrapartum Clindamycin Cream for Reduction of Group B Streptococcal Maternal and Neonatal Colonization

چکیده
OBJECTIVE In a randomized trial, we sought to determine whether 2% clindamycin cream administered intravaginally during labor to group B streptococcal-colonized pregnant women without risk factors would decrease maternal and neonatal colonization. METHODS The eligible women were randomized to receive either cream or no treatment. Two hours after treatment or admission, the patients were tested with rectal and vaginal cultures. The neonates of the study patients were also tested. RESULTS Of women randomized to cream, 5 of 5 remained positive and 2 (33%) of their 6 neonates were positive. Of 4 randomized to no treatment, 3 (75%) remained positive and 1 (25%) of 4 neonates was positive. CONCLUSIONS Intravaginal 2% clindamycin cream was ineffective in reducing colonization with group B streptococci.

Prevention of neonatal group B streptococcal infection.
Neonatal group B streptococcal infection is the primary cause of neonatal morbidity related to infection. It can often be prevented by identifying and treating pregnant women who carry group B streptococci or who are at highest risk of transmitting the bacteria to newborns. Increasing evidence and expert opinion support intrapartum treatment of women at relatively high risk of delivering an inf...
متن کاملPrevention of early-onset neonatal group B streptococcal disease.
Most cases of neonatal group B streptococcal disease with early onset have an intrapartum pathogenesis. Attack rates are increased substantially in infants born to mothers with prenatal group B streptococcal colonization and various perinatal risk factors (premature labor, prolonged membrane rupture, or intrapartum fever). In a randomized controlled trial, we studied the effect of selective int...

OBJECTIVE The purpose of this study was to determine the compliance rate with a maternal risk-factor-based guideline for the prevention of neonatal group B streptococcal (GBS) sepsis. METHODS In August 1994, a risk-factor-based guideline for selective intrapartum prophylaxis against neonatal GBS was adopted by a group model health maintenance organization. This guideline identified the follow...
متن کاملRelationship between Colonization of Group B Streptococcal (GBS) in the Reproductive System of Pregnant Women with PCR with Neonatal and Maternal Complications
Background and Aim: Group B Streptococcus (GBS) is a major contributor to maternal and neonatal death. GBS colonization in the mother can be transient or intermittent, which is why its examination during delivery is more valuable than screening at lower gestational age. The aim of this study was to compare the complications of premature delivery with term ones in positive cases of GBS by PCR me...
متن کاملAntibiotic Prevention for Maternal Group B Streptococcal Colonization on Neonatal GBS-Related Adverse Outcomes: A Meta-Analysis
Maternal colonization with group B Streptococcus (GBS) during pregnancy increases the risk of neonatal infection by vertical transmission. However, it remains unclear whether treating all colonized women during labor exposes a large number of their neonates to possible adverse effects without benefit. We performed a meta-analysis to assess the effect of intrapartum antibiotic prophylaxis on neo...
